Initial Assessment and Introduction to the Horses: $175 (2 hours)
Includes a two hour mental health assessment. You will meet the team and be introduced to the horses. We will focus on what brought you to therapy, gather information about your symptoms, review our office policies, explore limits of confidentiality, obtain consent, develop a diagnosis, and set your treatment goals.
Individual Rate: $150/hr ongoing services (50 min session)
Group Rate: $75 per individual for each session (90 minute session)

Insurance and Private Pay:
No Surprise Act and Good Faith Estimate:
Effective January 1, 2022, a ruling went into effect called the “No Surprises Act,” which requires mental health practitioners to provide a “Good Faith Estimate” (GFE) about out-of-network care to any patient who is uninsured or who is insured but does not plan to use their insurance benefits to pay for health care items and/or services.
The Good Faith Estimate works to show the cost of items and services that are reasonably expected for your mental health care needs for an item or service. The estimate is based on information known at the time the estimate was created. The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment.
You are entitled to receive this “Good Faith Estimate” of what the charges could be for psychotherapy services provided to you.
Fees for Trial, Court Ordered Appearances, and/or Litigation:
Rarely, but on occasion, a court will order a therapist to testify, be deposed, or appear in court for a matter relating to your treatment or case. In order to protect your confidentiality, we make every attempt to remain outside the courtroom. If we get called into court by you or your attorney, you will be charged a fee to include travel time, court time, preparing documents, potential costs of missed work.
$1200 per diem rate for each court day and additional $120/hr rate for the time spent preparing documentation for court.
Copies of Medical Records:
Should you request a copy of your medical records, the cost is $1 per page. Payment for your medical records will be due prior or upon receipt and can be picked up at the office. Please allow at least 2 weeks to prepare medical records.
Treatment Letters or Progress Reports:
Progress reports or treatment summary letters can be written at request of the responsible party at the rate of $75 per document.

FEES
Health Insurance varies greatly from plan to plan and will be subject to each individual’s out-of-pocket deductibles. All rates and fees will be discussed prior to the onset of treatment and you will receive proper notification if there are ever any changes to the scheduled fees.
PRIVATE PAYMENT: Payments are due in full at the time services are rendered and can be paid using cash, check, or credit/debit. Acceptable forms of payment are: exact-amount cash, check (insufficient-funds checks will be returned upon full payment of the original amount plus $25 for any returned check), or credit/debit card.
At Hearts of Freedom Wellness Ranch we use HIPAA compliant paperless systems for documentation of services and note taking. Within these systems we are able to accept debit/credit card payments in a secure manner through the client portal.
The clinician charges his/her hourly rate in quarter hours for phone calls over 10 minutes in length, email correspondence, reading assessments or evaluations, writing assessments or letters and collaborating with necessary professionals (with your permission) for continuity of care. All costs for services outside of session will be billed.
